I think that the problem with simplifying research terminology into layman’s terms is that it can create ambiguity about what actually occurs from a neuro-anatomical perspective. In the [hexagon](https://i.imgur.com/nTRVW6H.jpeg): For example, “behaviors” could be interpreted as the outward manifestations—i.e., blocks, repetitions, and prolongations—which reflect difficulties at the level of the speech muscles (respiratory, laryngeal, or articulatory) in executing planned words or sounds. Similarly, “emotions” could be interpreted as conditioned stimuli. For instance, feeling embarrassed (i.e., a conditioned stimulus - CS) is ultimately linked to a fear of social judgment or rejection. Through repeated reinforcement between speech execution and stimuli associated with fear of social judgements, the threshold for regulating speech execution may become poorly refined to excessively trigger a nervous system response (even when there is no conscious fear present). “Perception” could be interpreted as cognitive appraisal—identifying stimuli, evaluating them, and an interaction mechanism that respond to stimuli perceived as threatening for speech execution. When speaking in synchrony with a metronome or during choral reading, this evaluative process can often be bypassed, effectively addressing the perception element. “Beliefs” can be seen as the root of anticipatory patterns used to regulate speech execution. These patterns are based on prior experiences. Thus, even if choral reading temporarily modifies perception, long-held beliefs about the need to execute speech remain largely unchanged. If choral reading stops, stuttering often re-emerges, indicating that the underlying interaction mechanisms have not been successfully updated. Finally, “intention” can be interpreted as a willed intention—an internal neural commitment to initiate motor movement. Pre-motor execution requires a conscious, proximal, self-initiated “go” signal, an initiation signal that triggers the start of voluntary action before initiating the planned motor program. Neuro-anatomically, this is completely different from a subjective feeling of desire to speak. People who stutter often confuse these concepts, but executing speech does not require a feeling of desire—or any feeling at all.
