I used to have the same issue, for one year or more I wasn't able to say my name and surname. I was just paralyzed and if I managed to say it, it would have came out in hushed voice. Then this problem kinda disappeared since a few months, and I have no idea how that happened. I guess the only thing I did differently was not to avoid any kind of meeting where I was supposed to introduce myself, I tried to put myself in these situations despite I was struggling. Eventually it got better and better, and I almost don't get any blocks anymore whenever I have to say my name. It just come out so easily now, and that's such a wonderful sensation, considering that I was ruining my social life because of that. Just keep going and practice, it will get better eventually, the worst thing you can do is to avoid these situations.
