What I would do: After you walk into the room, say hello, shake their hands, sit down, they introduce themselves and then let you introduce yourself. You could start by saying, "Thank you for giving me this opportunity. I wanted you to know up front that... . My disability is that I can... this and that, but I'm confident that it won't affect the quality of my work." If they don't give you a chance to introduce yourself at the beginning, or they start asking questions too early, then answer the first question with what I wrote, it's not rude, it's rude if they never know what you wanted to say.
