This is how my stutter is these days too. What helps me is to make sure I take a deep diaphragm breath and start slowly exhaling before speaking. During my blocks, I feel lots on tension in my face, mouth, and throat so I focus on trying to relax during the exhale. I have to be mindful of it or I will quickly begin tensing up again.
