There's definitely merit to the dopamine hypothesis - hence current FDA medical trials of Ecopipam. What is interesting is where to go from here. This dopamine:stuttering hypothesis implies that there are ways to influence stuttering rates via diet and exposure to comfort/discomfort/pleasure/pain. The latter is a staple in both CBT/ACT/DCT-like therapies and most stuttering programs. The former, diet, is more interesting to me as I have noticed first hand links between stuttering and diet - beyond the obvious caffeine link. An interesting therapy approach would look like the following: 1. Train the PWS to reliably tune into and identify their dopaminergic state. 2. Influence their dopaminergic state via a number of techniques known to increase/decrease dopamine levels. 3. Repeat #2 iteratively with the intent of hardwiring new dopaminergic behavior.
