Lack of knowledge. When I had my daughter, they asked me while I was in labor a bunch of questions, including one about any communication difficulties. I told them I stuttered and found out a month or two later that the resident had used the icd code for intellectual disability. Soooo then I got to spend some maternity leave getting my medical record fixed.
