Depends. Sometimes I find it more practical to rephrase, but other times I feel like I really want to stick to what I had in mind. It varies. I wouldn't necessarily consider either of these the "truest". People in general use phrasing dynamically in delivering their sentences. We stutterers probably just tend to do it a lot more, out of need/habit. But as long as we've got some say in the matter, I don't see why it shouldn't be "true". Honestly, it's sometimes not a big deal to dodge problems like that. But if that's all you ever get to experience, that might feel like a problem. We shouldn't forget to focus on succes criteria for communication, though, namely that of being understood. And in that we have some leeway.
