Mine got a lot better when I moved from avoidance behavior to going on the offense. Basically feeling the fear but forcing myself into speaking situations. Allowing myself to stutter openly to reduce a lot of the associated shame. Lee Lovett has a book that has a lot of reviewers say they are "cured".
