"Reading in the head" isn't the same as reading out loud. That's why you're noticing a difference. Perhaps you have some sort of concentration difficulty when it comes to reading and imagining your voice at the same time. Does it change depending on the material? Like, complexity of the text, length of sentences, page denseness, that sort of thing? have you tried reading simple words one at a time, or short sentences on their own?
