I get that. If I wait and say a sentence in my head before saying it, it's way worse and I intensely focus on every word. If I'm caught off guard and have to speak, it's oftentimes completely fluent. It's odd. The less I try to focus, it gets worse again. Compete, natural lack of acknowledgement of my stutter is the only thing that really helps. Idk how to consistently do that though.
