Honestly I would still give Dr. Maguire's office a call and see what he has to say. He has colleagues in this field all over the world and can at least connect you with a like minded Psychiatrist or doctor to help. He could then guide them on what to prescribe to you with your medical history. The downside to Caplyta is that it's very expensive without insurance I think it's like $2,000 per month. I had to get prior authorization for it and that was a pain in the ass that took a month. If you wish to have a chat with me I would to share my history and help in any way that I can! The stuttering world is so small and we all have to help each other :)
