I also get where you're coming from. The terror I used to feel at times was.... indescribable. It's too bad therapy didn't help you. What I really took away from it were little tricks for sounds I would get hung up on. Eg: If 'd' was a hard sound for me I now start with a 'z'. Rather than door which can be difficult I'll say zdoor. People don't even notice the 'z' sound. For 't' I may start with an 's'. Etc etc, experiment with it. My therapist also encouraged me to 'add air' to my speaking. It's sort of like a breath from the back of my throat. It works well for 'h' and 'w' sounds. Maybe you need a better therapist.
