I always advocate for speech therapy to improve fluency. But I understand that different programs are better suited to different people. If someone is doing the work (and I mean working at it every day) and not progressing after a few months, it's time to change strategy. The program I went through didn't help my public fluency until close to the end of my therapy. But I was progressing through a program that was building towards fluency. Successes in the therapy office. Transitioning into fluency. Steadily improving. Practicing daily at home, by myself. Getting corrections and guidance at therapy.
