Dude, I also struggle a lot to say my name haha. The other day I went to Starbucks and when the guy asked for my name I just couldn't say it. But then when I'm alone I have absolutely no problem. In my case I think it comes from the added pressure of being something you HAVE to know. If I have trouble saying any other word I can just say I don't know, or pretend I'm trying to remember or to find the right words. If you don't know your name you look like an idiot which makes me feel specially anxious in these kind of situations. Also, you can anticipate the majority of times you will have to say your name like when meeting someone new or when calling to make a medical appointment. And when anticipating I tend to imagine myself getting stuck on that word which increases the chance of happening. I can't give you much advise as I also suffer from this. If you struggle with the first sound of your name you can try to start with an easy start like I'm... or My name is... to overcome the block. Or like Miss-Understood61 said breathing out while saying it also relieves a lot of pressure. But what I always end up doing is start with the typical uhmmmmm and then try to say my name even if it comes out badly pronounced. Not great but what can you do. Still working on it.
