I do the same as above comment mentioned. I know synonyms so well now that I'll just substitute a word I know I'm likely going to struggle saying. The more you practice the faster you can get with substituting and your block will be unnoticeable. I think we learn what kinds of words are most difficult for us over the years, but sometimes that list of words and sounds changes/evolves.
