I guess I need to preface my response with the fact that you seem much more educated on the subject than I do. I'm not familiar with some of the things you referred to by name. Likely have no experience with some of those things at all. I think the program that I went through did build resilience against stutter triggers. I also believe that through the practice and training that I received it did reduce the expectation of stuttering. When you can achieve an entire hour of fluency in the office, then proceed to continue to practice in private, and string a number of successes together in the office, that certainly reduces the expectation of stuttering. It was only after achieving those levels of successes and private that you start applying your fluency in the outside world.
