When it comes to fast food or take-out, I'll sometimes write down what I want and show it to the person I'm ordering from. Today I did that and the lady said it was easier that way, anyway (I was placing a carryout order from a restaurant). Seconding what others have said about sit-down restaurants - sit on the outside of the booth, so you can point at what you want. That's basically what I do.
