I think it's part anxiety, part learned behavior. Your brain has leaned over time to speak fluently in private , but in public "I'm a stutterer". I know which words I'll block on, but when I'm not actively thinking about my speech, sometimes I'll rattle off something and then realize I just said all those words without blocking. So "learned behaviors" is my vote.
