I feel this. Just to be solutions focused… do you think benzodiazepines would help in the short term for situations where you need to socialize or talk a lot? Maybe this could be effective? Maybe taking a short while might help rewire things. It sounds like talking off the cuff/spontaneously helps, as it does most people who stutter. I wish there was an easier way. EMDR therapy? Just throwing ideas out there. So sorry.
