Stop spending so much time and energy worrying about your stutter and just live. Stop the documentation, it's a sign of obsessiveness over your stuttering that is causing you to stutter more. Accept that you have a stutter and be open about it. Start small. Go to a restaurant (maybe one you won't visit again so it's risk-free), tell the cashier you stutter and then make your order. Then start making phone calls instead of emails where appropriate. Get on the phone as much as you can. Don't think about your stutter, just the task at hand. The more you think about your stutter the worse it will be. ​ Obviously this is highly subjective and different things work for different people but I am a big believer in Avoidance Reduction Therapy, and self-acceptance more generally.
