+1 for John Harrison. He has such a mature understanding of stuttering that was never presented to me in any speech therapist session. He was the first to introduce me to the concept of paradigm lock. His book is a goldmine of insights into stuttering. It's definitely a book to be paired with a more "action-oriented" program.
