I am curious if the rattling Whats are used to escape/back off from a sound he's afraid to stutter on or something else. Does he show other instances of stuttering? I would take him to the best stuttering trained therapist you can find. Most SLPs don't know much about stuttering so finding someone who does can really make a difference
