I start with voluntary stutter so people would wait if they encounter my blocks. sometimes I speak with sentence indicators if I anticipate worst blocks. "beginning of sentence. do or do... do not. end of sentence. new sentence. there... is no.... no try. end of my speech." I allow special type of interruptions, like repeating the sentence back to me after I say "end of sentence " but before new sentence.
