I'm the same in loud places. Speaking louder is more likely to lead to a stammer, for me anyway. This combined with memorising the order to say in advance and the fact the waiter/waitress is a new person makes it even harder. It has improved for me just by going to restaurants more as I started earning. I also use physical copies of the menu to point to whilst saying it as a help.
