I have the same problem, and both my last and first name start w/ the same consonant-vowel combination. But I only have this problem in social situations (introducing myself to strangers) or when I am on the phone. Although, when people ask me something like "Do you have a cold/did you just wake up/etc." and I tell them "No, I have a voice thing," it breaks up my block and then I can usually speak better.
