You don't have to order over the intercom at drive-thru. You could drive around and hand a note, detailing your order, to the person at the window. There is a sign on all fast food menus that those with hearing or speech difficulty are welcome to pull around in order to do this. The staff may be surprised at first but once they figure out what's going on they will accommodate you without issue. Going inside, same deal, just do the note. Bonus when dining in ... as they will likely assume you're hearing impaired, someone will bring your food instead of calling out your name/number.
