What's the difference between diaphragmatic and costal breathing?

What's the difference between diaphragmatic and costal breathing? I'm reading the book Beyond Stuttering and the costal breathing is one of the technique cited. But when I read the description I've got doubt with it and diaphragmatic breathing... What’s the difference? Also, should I always inhale and exhale by mouth in the costal breathing?
