my stutter is mostly a vocal block and it started due to anxiety and trauma. it comes at the start of words and starting slow and elongating words helps me sometimes. what Ive found really helpful is visualizing myself saying the sentence / word out loud fluently before speaking it. also looking up synonyms if you struggle with certain sounds helps a lot. I think we all get what your going through. it takes me a lot to even say my name and speaking to someone on the phone is especially hard for me to do. that doesn't even account for the metal gymnastics we have to do if we even think about saying something.
