If I'm with my wife, we'll discuss my order before, and if I have an issue I'll pretend like I forgot and she'll jump in with "weren't you gonna get" to bail me out. If I'm alone I'll still pretend like I forgot, kinda mumble and point at what I want like I'm looking for it. Half the time the server jumps in and I don't have to say much. At fast food/fast casual places I'm pretty fucked, so I try to avoid those. If I'm with a friend, I'll give them money and have them place one big order.
