Sometimes we can't be effortless. You can't just eliminate all tension or struggle right away. So depending on what you mean by effortless, I'd say we want to start with getting rid of all the things that don't belong in speech and then work on more cognitive-emotional parts such as a shame, letting people hear you stutter, letting yourself make sounds you don't want to make, and desensitizing you're own reactions
