I usually only had hard silent blocks (no repetitions). Right in middle of sentence, I’d suddenly look down and bang my hand on my thigh. Most people had no idea what I was doing. They only know if you tell them what you do. That’s why I think they have nervous/confused laughter - if you tell someone you might look down, pause etc and they laugh, then you can know they are laughing at you.
