I usually use a synonym when I know I have a block coming. When I'm in a presentation? I talk a bit slower, but if i get to a block, I take a second and breathe. There are plenty of methods to distract the people watching as you prepare yourself. My favorite method when I need to pause is I take off my glasses and take a few steps, say a few words, put them back on, then continue.
