Lovett's book may not be structured in an ideal way, but if you see it as a reference book for a toolbox of techniques that in some combination have worked very well for many others, it may be much more useful to you. He has a YouTube channel where he asks his students to demonstrate the crutches, which helps explain how to use them. Not all crutches will work, but the idea is to find which combinations that are most effective. For example, you may decide to skip a word in one sentence, and link words the next sentence. The key here is to divert your attention to something else, if the mere thought of speaking with someone triggers an automatic response (i.e. you may not stutter while alone since your attention is already elsewhere and not triggering anything). The sections for Auto Suggestions and Self Hypnosis are limited. The idea here is enforce that a repeated thought pattern is eventually perceived as reality. It follows the proven principle that Olympic athletes and piano players use to build muscle memory by imagining the desired result over and over again. The actual mechanism for this is brain plasticity, and several books exists on the topic. Auto Suggestions, Self Hypnosis, Visualizations, etc. are all simply ways of repetition and building neural pathways for a desired outcome. Some of his students have made miraculous changes as shown on his YouTube channels, and its no quick fix, but very helpful to be aware of all the tools at your disposal to slowly diminish the fear/trigger response.
