I would definitely work with an SLP but you need to make sure you're working with one who actually understands the issue. I would avoid SLPs who largely advocate operant or fluency centered methods of therapy. I really like ideas #2, 3, and 4 but I was be careful with what kind of "guided training exercises" you are suggesting. A lot of the most successful people who have overcome their stuttering/fear didnt focus on fluency. A lot didnt consider it at all actually. But that is where I think the right SLP would be able to help most. But if there's anything that keeps track of # of stutters or fluency, i think that's only going to do more harm than good.
