If you think anxiety triggers the blockages, look into some things you can do to reduce situational anxiety ( breathing meditation for example). I've the same problem... I cannot get my name out, and everything goes downhill from there. I do something radical though, which has worked beautifully for me (maybe 90% of time): I voluntarily stutter on my name and any other words that I usually block on. It works like a charm. I notice that 99.9% of people do not give a shit if I stutter or block. I do this regularly in front of 100+ audiences. Once I voluntarily stutter on those words, I slowly start feeling better, I start breathing easier, and my speech starts flowing properly. After the speech, the world doesn't end. I get good feedback for my speech, and all the fears and embarrassment turns out to be unfounded. Also, talk to your doctor about any 'situational' anxiety med that they might recommend, and give enough time to 'try' out the med before the important event.
