Desensitization by speaking in social situations more. Allowing myself to stutter/block freely without contorting my face grotesquely. Honestly just following a lot of the advice in "Self-Therapy for the Stutterer." The biggest shift in fluency came when I stopped caring so much whether or not I stuttered in public and stopped trying to hide it. This took years of work to actually happen.
