I have a neurogenic stutter (acquired brain injury) and suffer the same facial contortions. The way it was explained to me was the message is being sent from the brain to the vocal muscles but the signal gets messed up on the way down. The grimaces and straining are the result. I have never seen myself do it, but I have seen some people's reactions when I do it. Oh, my.
