This really hasn't been my experience at all. Any SLP worth their salt (at least when it comes to stuttering/fluency treatment) is going to include some amount of desensitization (which I think your points mostly fall under), since to the tension and anxiety reduction are "symbiotic" with the actual speech techniques. There's an entire school of fluency therapy that only focuses on the desensitization part, and pretty much skips the techniques entirely (I don't know how mainstream that is though). Unfortunately a lot of SLPs don't spend a lot of time on fluency in school, and might not be familiar with more holistic therapy approaches.
