Great other posts so far! My case sounds pretty similar to yours-- bad periods, good periods, and a name that's usually pretty tough. What I've found to work relatively consistently is saying my name without a "My name is..." or "I'm..." start. I can get sounds out well if I have a beat or cadence leading into it, either in my head or tapping my foot. So like "[beat] [beat] Paul" or "[tap] [tap] Paul" to get a rhythm going. Some people have success with stringing together connected phrases like "M y n a m e i s P a u l," but getting that first "My..." out is tricky for me, so I would still do the tapping anyway, haha.
