If it’s obvious enough that you’re disclosing it, then disclosing it isn’t what will make you lose the job. Disclosure just kills the elephant in the room, lets the other person know that you’re aware of it (as opposed to having a stroke without realizing it or something) and can be seen as being comfortable enough with it to not hinder you. Disclosure is almost always the best option.
